Discovery:
The first patent of finasteride was developed in the year 1984 and it was approved medically in the year 1992. Finasteride was developed by Merck. Merck had the initial approval from FDA in 1992 as Proscar for treating the enlarged prostate. The active ingredient of finasteride was initially useful for the treatment of hypertension and BPH (benign prostatic hyperplasia).
Mechanism of action:
Finasteride is the Type II 5α-reductase specific inhibitor, which is an intracellular enzyme capable of converting the androgen testosterone into the DHT. Among humans, the finasteride mechanism of action is based upon specific type-2 isozyme inhibition. In the human body, type-1 five alpha reductase is more prominent in the sebaceous glands, different regions of the skin, liver, and scalp. Type-one 5α-reductase is specifically responsible for the one-third circulation of DHT. For both of these isozymes, finasteride inhibition is associated with the reduction of dihydro finasteride inhibitors. Type II 5α-reductase inhibition helps in blocking the peripheral conversion of the male hormone testosterone into the DHT, which results in the significant decrease of concentrations of tissue DHT and serum.
Clinical applications:
Treatment of male pattern baldness:
This drug has specific indications for the treatment of male pattern baldness also known as androgenetic alopecia in which baldness starts at the crown and exceeds the scalp mid. The indication of finasteride is for adult men only. The mechanism behind this effect is the decrease in the DHT (natural body hormone). Decreasing the DHT concentration can lead to an increase in hair regrowth and eventually slower hair loss. Hair growth on the other body parts isn’t specifically affected by the consumption of finasteride.
Treatment of female baldness:
